Continuing Kare Model
Our services are available to patients who live in continuing care communities, including independent living, assisted living, and memory care settings. In these communities, most medical services are covered and reimbursed through insurance, allowing residents to receive high-quality primary care with minimal out-of-pocket expense.
Our team works closely with patients, families, and facility staff to ensure care is coordinated, accessible, and aligned with each individual’s health and living needs.
