Residential Kare Model
For patients who choose to age in place in their own homes, this care model includes an initiation fee and a monthly access fee. These fees support the additional time, travel, and coordination required to provide high-touch, personalized medical care in a private, stand-alone home setting.

While medical services are still billed to and reimbursed by insurance when applicable, the monthly fee ensures patients receive enhanced access, longer visits, and the convenience of in-home care tailored to their unique needs.
Fee Structure

Joining Fee (includes first month) $500
Monthly Access Fee $250
Payment Schedule
Fees are due on the 1st of each month and may be paid by Electronic Funds Transfer (EFT) or check.
Professional & Administrative Service Fees

Completion of History and Physical paperwork $350
Capacity Evaluations $850 Medical Testimony (per hour) $400 Form Fees (FMLA, LTC Forms, Letters) $100
