The fee structure for services:

$500     Joining fee and 1st month

$250     Monthly access fee

 

Fees are paid on the 1st of the

Month either by Electronic Funds Transfer or Check.

​​The patient’s insurance coverage is utilized to cover the below services when needed in the care plan:

•    Medical Visits

•    Telephone Evaluations

•    Home Health Care

•    Therapy Providers – PT. OT. ST

•    Laboratory –Sunrise Medical,       LabCorp, Quest, INOVA, VHC, ect. 

•    Specialty Care Providers

•    Advanced Primary Care Management

•    Collaborative Care Management
