Advanced Primary Care Management (APCM)
This Medicare-funded program is designed to provide structured, continuous, and proactive support alongside your primary care services.
It strengthens the connection between patients and their care team, helping individuals stay engaged with their providers, follow their treatment plans, and address health concerns early—before they become more complex. By offering consistent guidance and timely outreach, the program enhances care coordination and supports patients between office visits.
As a complement to primary care, this service promotes stronger relationships, greater confidence in care, and improved health outcomes, while ensuring patients feel supported, heard, and well cared for every step of the way.
Collaborative Care Management (CoCM)
An evidence-based, integrated care model that supports primary care by addressing common behavioral health conditions such as depression, anxiety, post-traumatic stress disorder, and alcohol or substance use disorders.

This approach works in close partnership with your primary care team, ensuring mental and emotional health needs are identified early, treated effectively, and seamlessly coordinated with your overall medical care—so patients receive compassionate, whole-person support.
