Offers vital support that helps patients stay connected to our providers, remain adherent to treatment plans, and address health issues early to prevent complications. This program improves patient engagement and builds trust, leading to better health outcomes and satisfaction
Collaborative Care Management (CoCM)
An evidence-based model of integrated care designed to treat common behavioral health conditions such as depression, anxiety, post-traumatic stress disorder, and alcohol or substance use disorders for patients.
